An alternative method offered only in conjunction with the purchase of the casket from Price Funeral Chapel, Inc., is to select one of our packages of services which are offered at significantly reduced rates.

The Price Funeral Chapel, Inc. respects cremation as an alternative to burial. We offer four alternatives for those considering cremation. For those who desire the traditional opportunities of visitation and chapel or church service prior to the cremation process, we recommend selecting our “Complete Funeral Service” or our “Basic Funeral Service” package. Those who desire visitation privileges, either private or public, and then the cremation process to occur and no services to be conducted, we recommend selecting our “Visitation Privileges” package and our “Memorial Service” package may be added. Those who desire a direct cremation with no visitation and a memorial service conducted without the decedent present, we recommend selecting our “Direct Cremation” and “Memorial Service” packages. Those who desire a direct cremation with no visitation and the decedent not be present for services, we recommend selecting our
“Direct Cremation” package.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This price list does not include prices for certain items that you may ask us to purchase for you, such as cemetery or crematory expenses, flowers and newspaper notices. The price for those items will be itemized on your “Statement of Funeral Services & Goods Selected”. A complete price list of caskets and outer burial containers will be provided by the Price Funeral Chapel, Inc.
We want you to be aware of the benefits of embalming. You should know that embalming provides sanitation, restoration of appearance and complexion and temporary preservation of the body. We welcome your questions regarding the preparation and care of the body. Chapter 12 of Title 16 of the California Administrative Code requires that a body held by the funeral director longer than 24 hours must be embalmed or refrigerated. The Price Funeral Chapel, Inc. offers both of these services.
If you want to arrange for an “Immediate Burial” or “Direct Cremation”, you can use an unfinished wood box or alternative container. Alternative containers are rigid, leak-proof and may be made of composition materials or heavy cardboard.
Traditionally, revenue for a mortuary has been generated from service fees and a modest margin on the casket sale. Families who elect to use an outside casket provider may select their services from our standard itemized pricing which reflect the true cost of providing those services without the revenue generated by the casket sale. Families who purchase their casket from Price Funeral Chapel, Inc. are entitled to the benefit of selecting from our packages of services which are offered at significantly reduced rates. Third party caskets must be delivered during regular business hours at least 24 hours in advance of scheduled a ceremony involving the casketed remains.
Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.

Our “Complete Funeral Service” package is offered only in conjunction with the purchase of the casket from Price Funeral Chapel, Inc. It is designed to simplify the arrangement process and to offer significant monetary savings compared to the itemized price list. This grouping of services includes all services listed on page 3 under the heading of “Basic Services Of Funeral Director & Overhead” plus standard removal from the place of death (removals available 24 hours per day, 7 days per week); physical care and preparation including washing; posing of the features; disinfecting; embalming and/or refrigeration; cosmetology; restorative art; autopsy restoration; hair styling; dressing; placement in the casket; evening use of slumber room for visitation; use of chapel facility for service (weekdays); staff and equipment for chapel or church service (weekdays) and/or graveside service (weekdays); audio equipment for graveside service; Cadillac funeral coach; Cadillac 8 passenger limousine; service van to transport flowers and service equipment; memorial register book, basic personalized memorial folders or basic personalized memory cards or basic personalized prayer cards, acknowledgment stationary; use of organ/piano and recorded background and vocal music. This special grouping of services is offered as a package and no adjustment is made for services not used.

Our “Basic Funeral Service” package is offered only in conjunction with the purchase of the casket from Price Funeral Chapel, Inc. It is designed to simplify the arrangement process and to offer significant monetary savings compared to the itemized price list. This grouping of services includes all services listed on page 3 under the heading of “Basic Services Of Funeral Director & Overhead” plus standard removal from the place of death (removals available 24 hours per day, 7 days per week); physical care and preparation including washing; posing of the features; disinfecting; embalming and/or refrigeration; cosmetology; restorative art; autopsy restoration; hair styling; dressing and placement in the casket. Your choice of either: A) use of slumber room or our chapel for an hour of visitation prior to service; use of staff and our chapel facility for service; use of organ/piano, recorded background and vocal music, audio tape of chapel service, use of service van for private delivery to cemetery or crematory; B) use of slumber room for an hour of visitation during business hours, use of Cadillac funeral coach, use of audio equipment and staff for graveside service. This special grouping of services is offered as a package and no adjustment is made for services not used.

Our “Forwarding Of A Decedent To Another Mortuary” package is designed to simplify the arrangement process and to offer significant monetary savings compared to the itemized price list. This grouping of services includes all services listed on page 3 under the heading of “Basic Services Of Funeral Director & Overhead” plus standard removal from the place of death (removals available 24 hours per day, 7 days per week); physical care and preparation including washing; posing of the features; disinfecting; embalming and/or refrigeration; autopsy restoration; cosmetology; hair styling; dressing; placement in the casket; identification by next of kin, correlation with common carrier and service van to local common carrier. This special grouping of services is offered as a package and no adjustment is made for services not used.

RECEIVING DECEDENT FROM ANOTHER MORTUARY
|
This grouping of services includes all services listed on page 3 under the heading of “Basic Services Of Funeral Director & Overhead” plus standard removal from Sacramento Airport or Sacramento Amtrak (available 24 hours per day, 7 days per week); disposing of bio-hazardous waste, cemetery/crematory correlation; service van for private delivery to cemetery or crematory. This special grouping of services is offered as a package and no adjustment is made for services not used.

DIRECT CREMATION
|
Complete Direct Cremation services range from $1,146 to $1,666. If you want to arrange a Direct Cremation, you can use an alternative container which encase the body, providing modesty and sanitary ease of handling, and can be made of materials like fiberboard or composition materials (with or without an outside covering). The Alternative Containers we provide are fiberboard or an unfinished wood box. A brief presentation is available providing consumer information regarding the various alternative containers and caskets offered for cremation and type of construction, features, benefits and qualities available. The following examples include our “Direct Cremation” package of services (listed in the following paragraph), county filing fee, crematory fee, sales tax and transfer urn: Direct cremation with alternative container provided by family is $1,146; Direct cremation including fiberboard container is $1,195; Direct cremation including an unfinished wood box is $1,623; Direct c remation including a covered & lined casket is $1,928.
Our “Direct Cremation” package of services includes all services listed above under the heading of “Basic Services Of Funeral Director & Staff” plus standard removal from the place of death (available 24 hours per day, 7 days per week); bacterial stabilization through refrigeration service for portion of the first and last day of care, placement in the casket or alternative container; service van to crematory and returning of cremains to Price Funeral Chapel, Inc. This special grouping of services is offered as a package and no adjustment is made for services not used.

MEMORIAL SERVICE

This grouping of services includes all services listed below under the heading of “Basic Services Of Funeral Director & Staff” plus use of chapel facility for service or staff and equipment for church service; use of organ and/or recorded organ and vocal music; preparation and delivery of newspaper notices; correlation with clergy.

BASIC SERVICES OF FUNERAL DIRECTOR & OVERHEAD
|
The Price Funeral Chapel, Inc. charges each family for our expertise in assisting them with their selection of appropriate services, facilities, equipment and staff. This fee includes sheltering of remains for portions of the first and last day under our care, disposing of bio-hazardous waste; a proportionate share of taxes; licenses; business office facility; insurance; 24 hour telephone availability; utilities, office arrangement conferences, correlation with cemetery/crematory, preparation and delivery of newspaper notices, clerical preparation of the death certificate and necessary permits and documents, death certificate services with local physician or coroner and registration with the county health department and other business related expenses required to serve those who call upon us, off-street parking, administrative and clerical services; consultation as appropriate with clergy, lodge and veterans organizations. (Assistance with Veterans Administration benefits and assist anc e with Social Security Administration benefits are provided without charge). The fee for our “Basic Services Of Funeral Director & Overhead” grouping of services will be added to the total cost of the funeral arrangements you select. (This fee is included in our charge for direct cremation, immediate burial and forwarding or receiving remains. It is also included in each of the services packages above.)

Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

For infants and children, the Price Funeral Chapel, Inc. extends special pricing considerations for services held on weekdays during regular business hours. For children 5 years and under, our service packages are 1/3 of those stated. For children 6-12 years of age, our services packages are 2/3 of those stated. The service packages for infants and children are noted in the table below. A complete selection of infant and children caskets are available and range from $95.00 to $3,500.00.

Our Standard removal is from an institution during regular business hours (Monday-Friday 8AM-5PM, except holidays) within 20 miles of Price Funeral Chapel, Inc.

PAYMENT POLICY
|
The Price Funeral Chapel, Inc. asks for settlement of the account prior to service time. Several options are available. You may select one, or a combination of, the following options:

- Pre-payment with our price guarantee
- Payment by cash or check from personal resources
- assignment of insurance policy benefits to Price Funeral Chapel, Inc. ($200 fee for processing of each insurance policy claim applies)
- Visa or Mastercard (4% convenience fee applies to cash advance items paid by credit card)

PRE-PAYMENT POLICY
|
The Price Funeral Chapel, Inc. welcomes pre-payment of funeral expenses which offers the benefit of our full price guarantee, regardless of inflation. There are no fees for this service and affordable payments may be made over as many as 10 years. Both the California Master Trust and Forethought Life Insurance plans are available. Please feel free to discuss the details with us.

FOR YOUR INFORMATION

To help maintain the cleanliness and environment of the Price Funeral Chapel, Inc, we do not permit smoking or refreshments inside the facilities. The Price Funeral Chapel, Inc. is not responsible for, nor will take any action to create liability for the serviceability or locking of a casket not provided by Price Funeral Chapel, Inc.

A WORD ABOUT GRAVE MEMORIALS
|
The Price Funeral Chapel, Inc. is pleased to offer a wide variety of grave memorials. Each grave memorial is installed at the cemetery of your choice with a concrete foundation and a galvanized steel flower vase. Our offerings include bronze memorials that may be installed flush with the surface or on a granite base. Our granite memorials may be installed flush with the surface or raised above ground level. Personalization can be achieved through a personal epitaph, color porcelain portraits and special artwork. Please feel free to call for an appointment to learn more about our grave memorials.

SCATTERING OF CREMAINS
|
The Price Funeral Chapel, Inc. recommends that families who prefer scattering of cremains make such arrangements directly with a licensed cremated remains disposer. The Department of Consumer Affairs (information noted below) can provide information about the cremated remains disposers they license.
